What Exosomes Are

Nanoscale messengers between cells.

Exosomes are 30-200 nm extracellular vesicles released by virtually every cell type — and prolifically by mesenchymal stem cells in culture. Inside each vesicle is a curated cargo: growth factors (VEGF, TGF-β, FGF), bioactive lipids, mRNA, and miRNA. When applied topically to micro-channeled skin or microneedled into the scalp, exosomes are taken up by local cells and deliver their signaling cargo intact. Research observes increased collagen and elastin synthesis, improved fibroblast activity, reactivation of dormant hair follicles, and modulation of local inflammation. Because exosomes contain no nucleus and no DNA, they offer a substantially preferable safety profile to whole-cell products: there is no possibility of unwanted cell division, no graft-vs-host concern, no oncogenic-cell-line worry. They are, in essence, the working instructions a stem cell would give the surrounding tissue — delivered as a packet rather than as the cell itself.

Skin rejuvenation and scalp regeneration.

On the skin side, exosomes pair with microneedling, Sylfirm X RF microneedling, fractional laser, or other channel-opening modalities to accelerate post-procedure repair and amplify outcomes. Indications include textural improvement, acne-scar remodelling, fine-line softening, post-inflammatory hyperpigmentation, sensitive or rosacea-prone skin, and post-procedural healing acceleration after laser or surgical work. On the scalp side, exosomes support early-to-mid-stage androgenetic alopecia (AGA), diffuse thinning, post-partum shedding, and stress-related telogen effluvium, offering more consistent dosing than PRP can deliver from a patient with age-related platelet count or quality decline. Both tracks typically run as a 3-session starter set spaced 4 weeks apart, with results emerging at 8-12 weeks and continuing to develop through month 6.

Lab-purified, controlled, traceable.

The largest practical question with exosome therapy is sourcing. The exosomes we use come from licensed cell-therapy manufacturers operating under defined good-manufacturing-practice protocols, with batch-level testing for purity, particle count, sterility, and absence of contamination. Each vial has a traceable batch number and a release certificate. We do not use unverified consumer-grade preparations marketed as 'skincare exosomes' that are not held to clinical sourcing standards, and we will tell you the manufacturer of the preparation you receive. Dr. Jiang's team evaluates each indication, confirms candidacy, and selects the application modality — topical post-microneedling, intradermal injection, or scalp-microneedled — based on what the tissue actually needs.

Exosomes vs. PRP — which is better?

Different tools, different best uses. PRP uses the patient's own blood and varies in concentration with age, medication, and venipuncture quality; it shines for orthopedic and tendinopathic indications. Exosomes are lab-purified stem-cell secretome at a consistent concentration; they shine for skin, hair, and post-procedure recovery. They can be sequenced or combined depending on the case. We will recommend based on your indication, not on what the clinic has more of in inventory.

Which kinds of hair loss are appropriate?

Early-to-mid-stage androgenetic alopecia (AGA Norwood 2-4 in men, Ludwig 1-2 in women), post-partum diffuse thinning, and stress-related telogen effluvium are the strongest indications. Severe end-stage hair loss with significant follicular miniaturization is not indicated — surgical hair-restoration consultation is more appropriate. We will tell you honestly which stage you are at.

Is exosome therapy approved by Health Canada?

Exosome preparations occupy an evolving regulatory space. We use preparations from licensed cell-therapy manufacturers operating under GMP protocols, with batch-level testing. What's the downtime?

Minimal for topical post-microneedling delivery — 24-72 hours of redness similar to a sunburn. Intradermal injection sites may show small bumps for 6-12 hours. Most members return to normal social activity within 24-48 hours and to exercise within 72 hours.

Insurance coverage?
